After a hospital stay
Coming home from Addenbrooke’s Hospital
Addenbrooke’s Hospital is on Hills Road in Cambridge and is run by Cambridge University Hospitals NHS Foundation Trust. If you or someone you care for has recently come home after a stay there, the first weeks can feel uncertain.
It is worth asking the ward team who will follow up, and when. You may be offered NHS community rehabilitation, or support from an early supported discharge or intermediate care team. What you receive depends on your needs, and we work alongside it. If you would like more practice at home, we can add to it.
It helps to have these to hand for our free call:
- Your discharge summary, if you have one
- A list of your medicines
- The name and number of your NHS contact, if you have one
