Complaints and feedback

Make a complaint or give feedback

If something has not gone as it should, we want to know. This page explains how to tell us, what happens next and where else you can take your concern.

Before you start

We would rather hear it and put it right

If you are unhappy with your care, a visit, our fees or the way we communicated, please tell us. We will listen, look into it properly and reply in writing.

Revival Neuro Physio is a small private service, and Rishee Patel is the only clinician, so your complaint will reach him directly. If you would rather raise it elsewhere, or you are not happy with our reply, the independent routes further down this page are open to you.

Not for emergencies. Call 999 if someone is in immediate danger or needs urgent medical help. For urgent medical advice that is not an emergency, call NHS 111.

What happens next

How we handle a complaint

Your care continues as normal while we look into it.

  1. You tell us

Use the form below, or call, text, message or email us. If writing is difficult, we can take your complaint by phone.

  2. We acknowledge it

Within 3 working days we confirm we have it. We then look into what happened, and may ask you for more detail.

  3. We reply in writing

Within 20 working days we send a full reply: what we found and what we will do. If the review is still going, we tell you why and when to expect it.

Complaint form

Tell us what happened

It helps to include:

  • your name and a way to reach you
  • what happened, and when
  • what you would like us to do

Share what you are comfortable sharing. We may ask for more by phone or in writing. You can ask a family member, carer or advocate to complain for you.

Prefer another way? Call, text or message us on 07777 793832, or email [email protected].

This form is not for emergencies. Call 999 straight away if someone is in danger or needs urgent medical help. For urgent advice that is not an emergency, call NHS 111.

Patient Complaint Form

Please provide details regarding your concern or experience so we can address it promptly.

We use what you send only to look into your complaint and reply to you.

Other routes

If you are not satisfied, or it is not about us

These organisations are independent of Revival Neuro Physio.

The HCPC

The regulator for physiotherapists. It looks at whether a professional’s conduct or competence puts people at risk or falls below its standards. It does not resolve complaints or arrange refunds or compensation.

NHS care

We cannot look into NHS care. Talk to the clinician or team involved, or ask that service for its PALS team, who can explain the NHS complaints process.

Questions

About complaints

If your question is not here, call, text or message us on 07777 793832.

Will making a complaint affect my care?

No. Your care continues as normal, and you will not be treated differently for raising a concern.

What if my complaint is about Rishee?

Rishee is the only clinician, so complaints reach him directly. If you would rather not raise it with us, or you are not happy with our reply, you can contact the HCPC at any time.

Can someone complain for me?

Yes. A family member, carer or advocate can complain on your behalf. We may ask you to confirm that you are happy for them to do so.

Can I complain anonymously?

You can, and we will read it. But we cannot reply or check details with you, so it is harder to look into.

What do you do with my complaint?

We use it to look into what happened and to reply to you. We keep a record of the complaint and our reply, and use what we learn to improve. We share details only with people who need them to deal with it.

Prefer to talk?

We are happy to talk it through

Call, text or message us on 07777 793832. We will listen, and tell you what happens next.

This page is general information, not medical advice. If you think someone is having a stroke or is seriously unwell, call 999. For urgent advice that is not an emergency, call NHS 111.
